MWSL Game Reschedule Request

This form is for the use of MWSL Team Representatives only. This form should be used to submit requests for a change in a game's field, date, or time (including games listed as TBD). Do not use this form to report scores or other concerns. Please use the Score Report form or contact the commissioner.

With the exception of some Field Manager changes, all reschedule requests received after May 14, 2003, are subject to a $10/game fine. In addition, all requests must be received 48 hours prior to the original game's scheduled time, failing to do so may result in additional fines to cover the referee fees.
